The following requirements and product information are valid only for the state of New York (excluding the 5 boroughs). Read the requirements and then select an ordering method from the Product Information table.
Requirements
The following requirements and reminders apply for the state of New York (excluding New York City). Please ensure that you have read and understand these requirements and that you have prepared any necessary documents and materials.
- Eligible requestors include self or parent listed on birth record ONLY.
- You must fax the requestor's official valid state-issued photo I.D. (Driver's License).
New York Birth Records will only process requests followed by a faxed copy of the requestor's government issued photo ID.
Enlarge the copy and lighten it as much as possible to ensure that it is clear and readable when faxed to Express Birth Records. - A signature under the copied ID is also required. Unsigned applications will not be processed.
- Photo ID address, billing address, and shipping address must all be the same.
THE NEW YORK STATE RECORDS OFFICE WILL ONLY SHIP THE CERTIFICATE TO THE CORRECT BILLING ADDRESS OF THE CREDIT CARD USED. - Due to the billing address issue, any pre-paid credit card, gift credit card, or a credit card not issued in the United States will not be accepted by the New York records office for an online order.

Ordering
You can order online or by fax. To order online, just click the "Online" link for the type of record you want and complete the online form. You must proceed through the Shopping Cart and receive your confirmation page in order for your request to be accepted.
To order by fax, click the "FAX" link for the type of record you want to get a printable application. Print the application, complete it, and fax it back to us toll-free at 877-684-5852 .
Product Information
The following prices are charged by the state of New York for the actual certificate and the shipping. The $19.95 fee that is charged by Express Birth Records will be a separate charge from the state's charge.
